I suspect bill Gates and his ilk think that programming is the code hacking which was fashionable when they actually dirty their hands with actual code. Nowadays, any organisation which deals with serious software has a legacy set of objects specific to their set of applications. In this context what renders the programmer effective is familiarity with those objects. For example a coder in computer animation would not get any credit for coding up quaternion algorithms in c++. That would have been done years ago. 'High level' languages such as C or C++ (I'm glad you mentioned FORTRAN, I thought only senile old fools like me used it), are the modern equivalents of assembler as far as what is actually required. Gates is expressing the frustration, which is common, that the 'high quality' staff he gets can't actually do the job. If he invested the two months or so in training new comers in the set of objects which underpins the set of applications which form the core business, he might find that the shortage will just evaporate.
